Verify a signed waiver
Every waiver signed online carries a verification code. Type it in below to confirm the copy you are holding is genuine, and which version of which waiver it is.

What this does and does not tell you.
It confirms that a signature with this code exists, which waiver and version it was
against, and when it was signed. It deliberately does not show the signer’s name,
contact details or answers — anybody can try a code, and a page that read those
back would be a way to go fishing for them. The office can confirm the rest.
